Lists oxygen concentrator, oxygen meter, and cage sets as individually priced items in an online-store format, with no separate "base fee" line item. For example, a 3-piece set ("Uniclear S5 + oxygen meter + Oxy Hotel L–LL size") is priced at ¥53,400 (tax included).
Whether this price is one-time or charged monthly isn't clear from the official site. A site notice referencing "customers continuing their rental into month 2 and beyond" describes an automatically-generated "auto order confirmation" email, which suggests a recurring monthly charge, but there's no explicit statement of "monthly" pricing. The FAQ does state "1 month from the day the rental unit arrives."

3. O2 Pet (O2ペット; operated by O2 Charge Co., Ltd.)
States clearly: "no base fee or contract fee — usable from the first month for the monthly rental fee alone." It's a flat monthly plan: ¥27,500 for 1 month, ¥22,000 for 3 months, ¥13,200 for 12 months — the longer the term, the lower the monthly rate. A dedicated cage costs ¥29,800 extra on the 1- and 3-month plans, and is free on the 12-month plan.
